A grind, whine, clunk, or vibration deserves attention, but sound alone does not prove that the transmission has failed. Noise can travel through a Sprinter’s body and may come from the transmission, driveshaft, differential, wheel bearings, brakes, engine or transmission mounts, or another rotating component.
Note when the sound occurs: in Park or Neutral, during a specific shift, while accelerating or coasting, only when turning, or at a certain road speed. Those details help a technician reproduce the concern and narrow the inspection.
